New Patient Paperwork

The first thing we will ask you to do is to complete some paperwork. This helps us get to know you and better understand the issues that have brought you to us. This includes a contact card, patient history and a review of your symptoms. If you’d like, you can fill these forms out in advance. Just print these out and complete at your convenience before your appointment. If you don’t get these done before your visit, don’t worry! Just plan to arrive 15 minutes before your appointment and you can complete them at this time.

Your Exam and Consultation

Your first appointment includes a full consultation with the doctor, as well as a comprehensive health screening and examination. The doctor will begin by reviewing your paperwork. Then you can discuss any symptoms, concerns and questions you may have. Depending on your specific condition, we may run additional diagnostics, including x-rays, to fully evaluate you. Depending on your specific condition, we may take X-rays to fully evaluate you.

Once all the information has been gathered and examinations have been performed, your doctor will give you a report of findings to answer the three most popular questions our patients have:

1. Can you help me?
2. What do you recommend that I do to get better?
3. How long will it take
4. How much is it going to cost?

After discussing your history and goals, examining your spine and reviewing any X-rays or additional tests, the chiropractor will discuss recommendations with you. If necessary, the doctor will then recommend treatment. We are an integrated practice, so If you have a condition that requires additional care or outside assistance, we will let you know and refer you to someone in our network of outstanding medical professionals. If a follow up appointment is needed, our front desk associate will assist you with that as well.

Wellness Orientation

The Wellness Orientation Workshop is a very important part of your care plan. It is required for all new patients. This new patient workshop informs you on how to get the most out of your care and reach your wellness goals. It explains the basics of your treatment plan and more about disease prevention.

Please speak with the Front Desk Associate at your office to sign-up for the next available class. Throughout the class, the treating doctor will go in-depth about undetected nerve damage, chemical toxicity, and adrenal stress. Guests, especially family members, are encouraged to attend.

Pricing and Payment

INSURANCE
Our office works with all insurance programs and providers. We also accept auto accident, personal injury, and worker’s compensation cases. Since there are so many plans and coverages, our staff will verify and explain your benefits at your report of findings. Be sure to bring your insurance card with you to your first appointment.

PRIVATE PAY
For patients who have little or no chiropractic coverage, flexible programs can be arranged. Discover Health and Wellness has many options for you to receive the care you need.

FAMILY PLANS
As a family wellness center, we encourage all members of your family to take advantage of the benefits chiropractic care offers. We work with you to come up with a customized plan for your family, one that is affordable and convenient.
